21 N. Park Street building occupants are listed below, and should use this form. An entity not listed below is considered a non-building occupant, and should complete the corresponding Non-Building Occupant form.

Event times wll be listed in the “Today at 21 N. Park St.” events listing. If request falls outside of Welcome Center hours, after-hours fees apply.

21 North Park Street Tenants/Building Occupants are automatically given a 1 hour window before and after their indicated start and end times to complete set up and tear down of the conference rooms. If the user has any special requests or reservation time concerns, please contact the Welcome Center Coordinator at 608-890-1559 or email at wccoordinator@uwmad.wisc.edu or add additional comments at the bottom of this reservation form.

Room set-up
  • 21 North Park Street Tenants/Building Occupants are responsible for their own conference room setups.
  • Welcome Center staff are available to answer any questions you may have in regards to conference room use.
  • At the conclusion of their event, 21 N. Park Street Tenants/Building Occupants are required to restore the conference rooms that have been used to their “default” setups. See the conference room guidelines for detailed instructions.
